A book of worst-case scenarios

I came across this article yesterday about US Rep Michael McCaul from Texas (who happens to chair the House Homeland Security committee) penning a book titled “Failures of Imagination: The Deadliest Threats to Our Homeland — and How to Thwart Them”.  The book, set to be published in January, will apparently outline a variety of terrorist attack scenarios against the US and how they can be stopped.

I’ve written in the past about the necessity to consider credible worst-case scenarios (natural and human-caused) which can impact your jurisdiction or organization.  Following the model outlined by Comprehensive Preparedness Guide (CPG) 201 for the Threat and Hazard Identification and Risk Assessment (THIRA) process, it’s not just enough to say you are vulnerable to flood or wild fire.  To identify what your specific vulnerabilities are (location, duration, severity), it is necessary to flesh these out into scenarios.  Identification of these vulnerabilities will then help identify impacts, such as those to infrastructure, resources, and populations.  Plans should then be based upon these impacts.

I’m doubtful that Rep McCaul’s book will provide any foundation for planning (although I don’t think it’s intended to), however the scenarios contained may be eye opening to EMHS professionals and even citizens.  Regular readers of this blog know I feel that we are on borrowed time regarding terrorist attacks.  Yes we have experienced some on US soil (and the UK, Canada, Australia, France, India, and many other nations), and they have been devastating, but we have to know that with organizations such as ISIS thriving on our planet, more will come.  Mumbai-type scenarios, with multiple coordinated simultaneous attacks can be crippling and certainly demonstrate what a credible worst-case scenario could look like.

I’m interested to see what Rep McCaul’s book contains.  I’ll be sure to publish a review once it comes out and I’ve had an opportunity to read it.

Are You Really Considering All Hazards?

Natural hazards, such as flooding, tornados, wildfire, and earthquakes, bring about the greatest losses, calculated in nearly every metric possible, as compared to human-caused incidents.  Human-caused incidents, either accidental or intentional, still bring tremendous impact to communities world-wide on a daily basis.  While working to prepare for, mitigate, respond to, and recover from natural hazards will always continue to be important, it seems that many still often forget about human-caused incidents despite all the conversations out there.

Human-caused incidents include a variety of hazards such as infrastructure failure, transportation accidents, hazardous materials incidents, and intentional attacks.  These are all things which we can fit into our traditional model of Prepare, Mitigate, Respond, and Recover.  The National Planning Goal introduced the model of the five Mission Areas – Prevention, Protection, Mitigation, Response, and Recovery – to help address our many of our major functions (Core Capabilities) for human-caused incidents (note that Preparedness is now a higher level concept that applies to all Mission Areas).  While this Mission Area model has helped bring these key activities into the greater fold of what we do, it has also kept them largely isolated through the thought that many human-caused incidents are only addressed through Prevention and Protection Mission Area activities.

Nowhere, it seems, do we see this more than in the area of hazard mitigation.  The vast majority of hazard mitigation plans which exist only address natural hazards (even at the state level).  Since many readers view this blog for my opinion, here it is – this is archaic and dangerous thinking!  We have all seen hazard mitigation plans which claim they are ‘all hazards’, yet only list natural hazards.  That’s fine, if by some unbelievable circumstance, your jurisdiction is only impacted by natural hazards.  This is a circumstance which I am highly doubtful of.  Some mitigation plans get a little more realistic and will address human-caused hazards such as dam failure and/or hazardous materials release, which were likely the greatest human-caused threats they may have been vulnerable to in the previous century.  In today’s world this still doesn’t quite get us to where we need to be.  There are a great many mitigation activities which we can leverage against human-caused incidents.

How do we fix this?  It’s easy – start with conducting a hazard analysis.  A hazard analysis, be it as a stand-alone activity or part of the THIRA process, should review all possible hazards which your jurisdiction, company, or organization is vulnerable to.  It should be comprehensive, not just limited to the set of natural hazards.  Along with infrastructure failure and hazardous materials incidents (both in-transit and fixed site), consider hazards such as active shooters, cyber attacks, improvised explosives, and civil unrest.  This may require bringing some additional subject matter experts into the room for your hazard analysis – like your IT director.  In a hazard analysis, each hazard is ranked (at a minimum) by its likelihood to occur and its severity of impact should it occur.

A well conducted hazard analysis provides the basis for everything we do in emergency management and homeland security.  It not only informs our activities such as planning, training, and exercises, it also helps assign priority to those hazards which require the greatest focus and allocation of resources.

Emergency Managers Need to be More Like Engineers and Less Like Shopkeepers

I was inspired by this short (~1 minute) video from TrainingJournal.com.  In the presenter’s brief but pointed message, he describes many trainers as being akin to shopkeepers, providing organizations often times with rote solutions just as a shopkeeper will pull a product off their shelves. He goes on to say that this these solutions are usually effective, but only for a limited duration.  He offers, instead, that trainers need to be more like engineers, examining every facet of a problem and constructing lasting solutions.  As an experienced trainer and proponent of a detailed root cause analysis, I couldn’t agree more, but as I readied myself to write a post about the implications of this on training, my mind carried this metaphor to many of our practices in emergency management.

Consider how often we quickly dismiss identified gaps with an assumed solution.  Write a plan, conduct a training, install a bigger culvert.  Those are usually our solutions to an identified problem.  Are they wrong?  No – we’re correct more often than not.  Are these lasting solutions?  Rarely!  How often does the problem rear its head again within a relatively short span of time?  How do we address the re-occurrence?  As shop keepers we simply pull another solution off the shelf.  Can we do better?

The things we do in emergency management are often based upon best and current practices.  We address problems through the prevalent way of dealing with such things industry-wide.  Emergency management has a great community of practice.  I’ve mentioned in several previous blog posts the spirit of sharing we have and the benefits we see come of that.  It doesn’t seem often, though, that we engage in an industry-wide groupthink to solve various problems.  We use and adapt ideas of individuals and small groups, we see a steady and determined progression of the practices within our progression, but we rarely see ‘game changing’ ideas that revolutionize how prevent, prepare for, respond to, or recover from disasters.  Why is this?

Perhaps we need a greater collective voice locally, where practitioners are dealing with the problems directly?  Our methods of practice in emergency management are generally driven by the federal government (THIRA, NIMS, HSEEP, etc.).  I’m not saying any of these are bad – in fact they are excellent standards that we need to continue to refine and apply, but it’s generally not the federal government that is dealing directly with the constant flow of issues being dealt with at a state, and even more so, a local level.  We need to follow that metaphor of being engineers to apply more permanent solutions to these problems.  We need to create, innovate, and problem solve. Or do we?

Necessity, as they say, is the mother of invention.  We often miss the necessity of improving because we have current, functional solutions – we have things that work.  So why fix it if it’s not broken?  I say we can do better.  The realization of the need for lasting solutions is the necessity we need.  If the solutions we have on the shelf don’t work for us 100%, let’s figure out a better way.

I don’t know what or how, but I’m sure that as a community we can identify needs and prioritize what must be addressed.  Given the right people, time, and maybe a bit of money, we can be innovative and effect some lasting change.

Best Practices for the New Year – Resolve to be Responsible, Realistic, and Resourceful

As I work with jurisdictions and discuss their capabilities I find a broad range of perception among emergency managers about their jurisdictions’ capabilities and limitations.  Some overestimate their capability, thinking that they can handle anything and don’t need any outside assistance.  Others underestimate their capabilities, with their emergency plans defaulting to calling for help or making someone else responsible for nearly every scenario.  Fortunately some jurisdictions are spot on and have an informed and realistic perception of their capabilities.  Having the wrong awareness of what your jurisdiction can and cannot do can be dangerous.

Be Responsible

First of all, jurisdictions need to be responsible for their people.  Far too often I see an automatic assumption that someone else will handle an incident or a certain aspect of an incident, apparently abrogating the jurisdiction of all responsibility.  One of the more common occurrences of this is with sheltering where I rather often hear ‘The Red Cross will take care of that.’ with no further discussion even considered on the subject.  With no slight intended toward the Red Cross, relying on one entity to provide an absolutely critical capability is simply foolish.  If the Red Cross or any other outside entity is for some reason unable to provide these services for the jurisdiction, the jurisdiction is still left with the responsibility to provide this care for its citizens.  A jurisdiction without a plan to address this need is not being responsible for the welfare of its citizens.

The primary goal of a jurisdiction is to provide for its citizens.  Take this seriously and remember that you can’t assign this responsibility to others.

Be Realistic

Know your capabilities and your capacity.  In other words, know what you can and can’t do; and for what you can do, know how well and how long you can do it for.  Know what your limitations and dependencies are.  If your jurisdiction’s ability to provide advanced life support (ALS) care is dependent upon the only paramedic you have as a member of your ambulance service, you have very little capacity and quite a bit of vulnerability.

A good start to having a realistic view of your jurisdiction’s capabilities is conducting and regularly updating a comprehensive threat and hazard identification and risk assessment (THIRA).  THIRA is an in depth assessment which combines a traditional hazard analysis with a reference to DHS’ 31 Core Capabilities in the context of the threats specific to a jurisdiction.  I strongly suggest that a jurisdiction conducting a THIRA extend this assessment into an analysis of five key elements (Planning, Organizing, Equipping, Training, and Exercising – POETE) for each of their capabilities.  Go here for my post on the POETE analysis which explains the benefits and the process a little more.

A good THIRA helps jurisdictions identify not only their hazards but also the potential worst-case scenario impacts of these hazards.  It then provides an opportunity for the stakeholders of the jurisdiction to take an honest look at their capabilities and their ability to leverage these capabilities against those impacts.  Being honest in this assessment will help jurisdictions see what can hurt them most and identify the gaps and limitations they have in their capabilities.

Bottom line – be realistic in what you can do, how well, and how long you can do it.

Be Resourceful

The ability to endure the impacts of a disaster and, at a minimum, address the critical objectives of life safety, incident stabilization, and property conservation can require a jurisdiction to be creative and resourceful.  This is a key aspect of resiliency.  While assistance may still be needed from outside sources, a jurisdiction’s ability to survive and provide lifeline services for its citizens in the interim is extremely important.  Being resourceful can help a jurisdiction shore up its capabilities in times of need.  Key to being resourceful are good contacts and connections within the whole community.  Religious groups and social organizations, private companies, and even individual citizens can all provide services which can aid a jurisdiction in shoring up capabilities – at least in the short term.  Incorporate these as options within your emergency plans.  While these entities may have issues and commitments of their own during a disaster, they may also be able to help.

Use all available resources to get the job done and to sustain for as long as you can.  It can absolutely be the difference between life and death.

An old boss of mine once told me that to find the real root of any problem you should ask ‘Why?’ five times.  This sage Yoda-like advice has served me well ever since.  Of course it’s not always necessary to ask it the full five times; in fact you often find the foundational cause sooner.  Nonetheless, this approach will inevitably guide you toward discovering what needs to be fixed.

Those who follow my blog know that I post mostly within two thematic areas – emergency management or training.  The ‘ask why’ methodology applies to both of these areas and darn near anything else I can think of.  My thoughts are below on both themes.  Of course training in the field of emergency management is a combination of the two!

In Emergency Management

I’ve posted numerous times on topics such as hazard analysis, Threat and Hazard Identification and Risk Assessment (THIRA), and other similar topics in emergency management.  It is so incredibly necessary for us to identify needs and vulnerabilities, and to understand our community’s capabilities in order to properly prepare for future disasters and emergencies.  I’ve learned that in public safety, when asking a question, you often get a story and that story is often related to a past incident.  While the story may be elaborate, it usually gives you little substance.  Anecdotes aren’t enough.  You need to dig deeper.

As a culture within public safety we are still trying to drive practitioners to be more analytical.  Quality after action reports are a big step in the right direction.  The benefits of after action reports for incidents, not just exercises, are huge.  After action reports should lead to improvement plans, but without identifying the real reason behind what went wrong we can’t fix the problems.  After action reports require an analysis to dig deeper into the observed action to discover what really needs to be addressed.

In Training

In November I published an article in Training Magazine titled The Importance of Analysis to Identify Root Cause.  While I didn’t reference the ‘ask why’ methodology directly, the subject matter of the article lends itself to this approach.  As a trainer, when a problem is presented to you to ‘fix with training’, you need to figure out what the real issues are so that 1) you can confirm that it is in fact a training issue, and 2) you can determine what the objectives and methodologies of the training need to be.  Without properly identifying and defining the needs you are doomed to fail and will likely be putting forth a lot of effort with little gain.  While the results may put some people on the defensive, they can point the organization in the right direction to address inefficiencies and performance problems.

In any needs assessment, don’t simply accept the first answer given to you – dig deeper!  It’s amazing what you will find!

Examining Needs-Based Emergency Planning

For the past decade and one half we have seen documents such as Civil Preparedness Guide (CPG) 1-8 (1990), State and Local Guide (SLG) 101 (1996), and two versions of Comprehensive Preparedness Guide (CPG) 101 (2009 and 2010) provide us with continually advancing standards and guidance for emergency planning.   We have seen the focus points of planning evolve from assumption-based, to threat and risk-based, to capability-based planning through each of these iterations.  With the release of each new standard, however, the lessons learned from the previous have been preserved, bringing with them remnants of the earlier standards.  Our current standard, Comprehensive Preparedness Guide 101 (2010), maintains a focus on capability-based planning but still stresses the importance of formulating assumptions in our planning as well as identifying threats and risks.  Each of these elements is important, but in these examinations we seem to be forgetting something very important – what is the need?

Planning assumptions, risk and threat, and capabilities assessments are all important informers of emergency planning and must remain in the lexicon for us to be successful.  It seems, though, that while these elements contribute to our planning efforts, they still don’t define the true need.  In examining the real need in any jurisdiction, we need to identify these other elements but we can’t take the jurisdiction itself for granted.  Identifying the needs of the jurisdiction will help us, along with the other elements, to identify what the impacts of a disaster will be and how prepared we are to address them.  Too often we see emergency planning efforts which are very rote, paying little attention to the real needs of the jurisdiction.

If you have followed my blog for any length of time, you likely recall that I am a huge proponent of needs assessments.  As a trainer, a proper needs assessment is everything.  It leads us to the identification of what the desired behavior is and is a critical first step in determining how we will effectively train individuals to achieve it.  Earlier this month I had an article published in Training Magazine on the Importance of Analysis to Identify Root Cause.  The same principles of needs assessment can be easily applied to emergency planning.  Very simply, needs drive objectives.

The identification of needs for a jurisdiction involves an examination of both the physicality of the jurisdiction as well as the population.  Elements of the physicality of the jurisdiction include size and geography, accessibility of areas within the jurisdiction, and critical infrastructure and key resources contained within the jurisdiction.  Examining the population demographics includes age ranges, income levels, disability, vulnerable and at risk populations (the CDC Social Vulnerability Index is a great resource), languages, cultures, religions, population densities, and the ratios of full time residents to transients/visitors, and commuters.  GIS can provide us with much of this information both individually and in aggregate.

Once we collect this data, an analysis is important to identify what it all means (aka defining the need).  Where are there vulnerabilities within the jurisdiction in a steady state?  Under which scenarios exist increased vulnerabilities – such as a bridge that provides the only access to an area of the jurisdiction being washed out.  What religious and cultural matters must be considered in disaster response?  What needs exist for communicating with those with limited English proficiency?  The answers to these questions will inform strategies contained in our emergency plans and annexes.

Good planners dig to these depths and produce quality operational plans – but most don’t.  Plans which have not been written with this detailed process are doomed to fail as the needs of the jurisdiction have not been weighed with our assumptions, threats and risks, and capabilities.  The THIRA process helps to move us in the right direction by asking us to provide threats and hazards with context (our planning assumptions) and then establishing capability targets which will address these impacts.  Still, it’s not direct or detailed enough to provide us with all the information we need.

While CPG 101 guides us to know our communities and to understand the consequences of a potential incident, the current focus on capabilities, while important, is a focus on us – public safety.  The focus must be on the jurisdiction as a whole and an identification and understanding of potential impacts and the resultant needs of the jurisdiction.  It’s not so much a change in process as it is a change in emphasis.  We must first understand needs before we can plan to address them.

Capability Prioritization

The THIRA (Threat and Hazard Identification and Risk Assessment) process is the most comprehensive hazard analysis in emergency management and homeland security.  It provides more information than the traditional hazard analysis by examining each hazard through the lens of each of the 31 Core Capabilities which have been specifically defined by the stakeholders of the jurisdiction.  The end result of the THIRA is a snapshot of the hazards a jurisdiction faces and the identification of what is needed to handle that hazard within each of the five mission areas (Prevention, Protection, Response, Recovery, and Mitigation).  It can be a complex process, but well worth the investment of time for a jurisdiction.  More, however, can be learned and the THIRA should inform more than just your plans.

For states and UASIs (Urban Area Security Initiative jurisdictions), the process continues in the form of the SPR (State Preparedness Report).  The SPR uses the THIRA data and applies a POETE (Planning, Organizing, Equipping, Training, and Exercising) analysis to each capability.  The POETE analysis drills deeper into each capability, allowing the jurisdiction to better understand their strengths and weaknesses within each capability.  This is extremely valuable information and clearly there are benefits to more than just states and UASIs conducting a POETE analysis.  The SPR process also prompts jurisdictions to assign a priority to each capability – High, Medium, or Low.  All in all, this provides a depth of data, but what does it all mean?

While the SPR process expands on the THIRA foundation by prompting a more in-depth analysis of each capability, the end result is a multitude of data points.  Taken individually, a jurisdiction can examine details of a specific capability, but further analysis needs to be undertaken to see the big picture.  Many jurisdictions rate quite a few of the 31 Core Capabilities as a High priority.  So what do you focus on?  If everything is a priority then nothing is a priority!

What’s the (next) Big Idea in Emergency Management?

Innovation.  It seems to be what everyone clamors for.  In emergency management we see people striving for it across the board: in government and in education we try to build the better emergency management mouse trap.  We establish think tanks to find new solutions and the private sector looks for better ways to protect their investments.  But what is it that we are looking for?  What systemic problems do we still face in emergency management that require change? 

There is plenty out there that needs to be improved upon.  There always will be.  Until we can prepare for, prevent, and mitigate disasters to the point that little to no response is ever needed and no loss of life occurs we will continue to strive for better ways of doing things.  I’m guessing that day is a long way off, so we have plenty of work to do.  Before we can innovate, however, we must find cause.  Necessity, as they say, is the mother of invention.  So what needs exist that must be corrected? 

Certainly our after action reports (AARs) identify areas of needed change.  But those generally only show us gaps in local systems.  Threat and Hazard Identification and Risk Analysis (THIRA) likewise shows gaps in local systems.  Does this information ever get fed to higher levels?  Of course it does… in some measure but only some of the time.  States assemble State Preparedness Reports (SPRs) which, in current practice, conduct an analysis of each core capability through each of the POETE elements (planning, organizing, equipping, training, and exercising).  These in turn inform the National Preparedness Report (NPR).  The 2014 NPR was released by FEMA earlier this month, identifying areas for improvement in several of the core capabilities.  This is certainly a resource to help us identify needs, but none of these resources or mechanisms are perfect.  What is missing?  How do we improve them?

Interestingly enough, some opine that we aren’t examining the right data.  The Congressional Research Service suggests that we might need better measures of preparedness, according to their report and this article from FierceHomelandSecurity.com.  The report gives no answers, but poses several questions.  Overall, what can we do better?

Returning to innovation, where do the gaps truly exist?  How do we validate those gaps?  Can we address those gaps with current systems or do we need to create new systems (innovations)?  If it is with current systems, what are the barriers to getting the gaps addressed in the short term?  If it is not with current systems where does the innovation come from? 

Despite having worked in Emergency Management for over fifteen years and having seen, felt, and experienced the myriad changes which have occurred – especially since 9/11 – and with every administration subsequent to the attacks I really hadn’t sat and considered the changes that have occurred.  I’m about half way through an amazing book by John Fass Morton called Next-Generation Homeland Security: Network Federalism and the Course to National Preparedness.  The first 200 pages or so of the book provide a thorough review of civil defense/emergency management/homeland security through decades and over a dozen presidential administrations.  The gravity of it all has left my head spinning.  So many changes – and most simply for the sake of politics.  Much of it seems like wasted effort, but Mr. Morton connects the dots so brilliantly and identifies that D certainly could not have happened if not for A, B, and C… even though C and A were essentially the same.  IT seems that through these years so much has occurred, but so little has actually changed.  I would argue that the practice of emergency management is in a better place now than ever, but what will emergency management look like tomorrow?  Will our continued evolution be through measured change or through innovation?  What makes that determination?

Keeping Up With Changes in Emergency Management

As with many things in life, the field of emergency management has changed and will continue to do so.  Much of this change is an evolution – generally positive and productive adjustments to make us more effective and efficient in what we do supported by doctrine and models to guide our actions and provide consistency of application.  Sometimes changes are made which simply give the illusion of progress or are applied much like a Band-Aid as a knee-jerk stop-gap measure which usually fail unless a better implementation is put in place.  Many of the better thought out applications, however, do tend to stick.  While we have seen a great deal of change in the field over the last 14 years, we have largely seen a clear progression with practitioners and policy makers learning from previous programs. 

Yesterday I encountered two separate instances which did not apply current practices and policies. The first was an advertisement for a training program which discussed the four phases of emergency management.  The second was an article in which the author stated that ‘…preparedness is no longer part of the (emergency management) lexicon…’.  The two items, while different, are related in that they both indicate a lack of understanding in the evolution we have made from the four phases of emergency management – mitigation, preparedness, response, and recovery. 

In a nutshell, these long standing phases began to change soon after the terrorist attacks of 9/11 with the integration of homeland security with emergency management resulting in the inclusion of ‘prevention’ into the emergency management phases – thus prevention, mitigation, preparedness, response, and recovery.  As minor as this seems, it was quite a change for those of us who had been in the world of the four phases for a while and was a difficult pill to swallow.  Along with the human nature of resistance to change there was still a feeling that the matter wasn’t quite fully settled – in other words, more change would come. 

For several years different models were kicked around but none really gained traction until the issuance of Presidential Policy Directive 8 (PPD8), which created the National Preparedness Goal and the National Preparedness System.  These begat things like the Core Capabilities (a revamping of the predecessor Target Capabilities) and the introduction of the Threat and Hazard Identification and Risk Assessment (THIRA) as well as a new way of viewing the major activities within emergency management and homeland security – the five mission areas of prevention, protection, mitigation, response, and recovery.  These five mission areas have re-defined, or perhaps more accurately defined what it is that we do in emergency management and homeland security. 

The traditional four phases were often depicted in a cycle.  Taken literally, this meant that you progressed from one phase to the next in a series.  The truth of the matter was that each of the four phases could actually run simultaneously.  There was also a misunderstanding that preparedness was an isolated activity, when in actuality our preparedness efforts applied to all activities.  With the further evolution of homeland security the foundational activities of prevention and then protection were identified and defined.  Pulling together these five mission areas – prevention, protection, mitigation, response, and recovery – the National Preparedness System provides for distinct preparedness activities identified for each mission area, an organization of the Core Capabilities within each mission area, and national planning frameworks which identify the role and goals of each mission area in achieving the national preparedness goal.  Not only has preparedness not gone away, but it has been elevated in status. 

PPD8 was probably the presidential directive with the greatest and broadest impact on our field of practice since Homeland Security Presidential Directive 5 (HSPD5) in 2003 which drove the implementation of the National Incident Management System (NIMS).  Keeping up with critical changes in our evolution such as these is absolutely imperative for practitioners.  Not only do these policy changes impact how we do our jobs individually and programmatically, but they impact how we coordinate with each other, which is and always will be the foundational essence of emergency management.
